KMPDC/COC/KPA etc, Business Registration certificate, Copy of KRA PIN certificate, Valid Tax compliance certificate, Copy of valid Professional indemnity License, Id card/Passport copies of the shareholders, Certificate of Incorporation and Company CR12(less than 6 months old.

(for limited company)

• Certificate of change of name (If applicable), Comprehensive pricelist in excel format, SHA certificate (providers offering inpatient services), Trustees deed/Societies rule/NGO, Certificate/Certificate of exemption (If applicable)

Mandatory required if provider is offering the service

• Pharmacy licenses i.e Pharmacy and poisons board, Laboratory licenses i.e KMLTTB, Radiology licenses i.e Radiation protection board
